Referral partners

Refer someone to Footsteps

This pathway is for courts, probation departments, attorneys, clinics, schools, EAPs, community organizations, and other authorized referral sources coordinating services for someone else. Access is limited to information appropriate to the referral relationship.

What type of referral partner are you?

Choose the role that best describes your referral relationship so the request reaches the right team. Please leave diagnoses, medications, therapy details, and other sensitive case information out of this form. A Footsteps staff member will follow up securely if additional information is needed.

Keep public forms non-clinical

Do not include protected health information, diagnoses, medications, clinical details, court documents, or sensitive case information. Requests are routed to Footsteps operations for review.

What to expect after you refer

  1. 1. We confirm receipt and provide a reference number.
  2. 2. A Footsteps staff member reviews the referral and its requirements.
  3. 3. We contact the referred person and confirm an appropriate next step.
  4. 4. You receive status and completion information you are authorized to receive.

Footsteps determines service fit and enrollment eligibility. A referring court, probation department, agency, employer, or other authority determines whether a Footsteps program or completion document satisfies its own requirements.
